According to the Centers for Disease Control (CDC) over 30 million Americans, almost ten percent of the U.S. population, have diabetes. Currently diabetes is the seventh leading cause of death in the United States, and individuals with type 2 diabetes are at increased risk of heart attack, stroke, blindness, kidney failure, and loss of toes, feet or legs.

Valley Health's Diabetes Management Program

Although diabetes is a serious health condition, it is one that patients can manage and learn to live well. Living with diabetes requires education and a patient’s willingness to participate in his or her own health. With pro-active lifestyle changes, patients can prevent or delay the complications that arise from diabetes and hyperglycemia. Valley Health’s diabetes educators, dietitians, and nurses can provide education and support to help people manage their disease on a daily basis. This puts patients in control again, allowing them to lead healthy and fulfilling lives while managing their disease.

At Valley Health, we take our patients’ support seriously. That’s why we have designed a fully-compliant and robust diabetes management program, which has been recognized by the American Diabetes Association since 1993 for the quality of our education program. We provide support and resources through individual and group sessions.

Prediabetes

Have you been diagnosed with higher-than-normal blood sugar, also known as prediabetes? The Diabetes Prevention Program is a FREE program for adults at risk for type 2 diabetes that is designed to help you take steps to decrease your risk of both diabetes and heart disease. Both group and one-on-one support help are available.

Participants will learn about healthy eating to safely lose weight; how to slowly and safely increase physical activity; ways to problem solve and stay motivated. Offered through Valley Health Wellness Services, this lifestyle change program is ideal for those who are ready to make a lasting change, and is recommended and open to anyone who wants to understand how to improve their health.
