
The DAISY Award is an international recognition program created by the family of J. Patrick Barnes. Anyone may submit a nomination - patients, family members, visitors, and co-workers - who experiences or observes extraordinary compassionate care from a Valley Health Registered Nurse (RN). Awards are presented annually.
You can choose one of two ways to recognize an RN:
The Carnation Care Award is a quarterly award given to an outstanding Certified Nursing Assistant (CNA) or Technician (Tech) that demonstrates consistent qualities above their expected performance. Anyone may submit a nomination - patients, family members, visitors, and co-workers.

Send completed form to the appropriate person. These are the same individuals as referenced above for the Daisy Award.
The PHIL Award is given annually to one Respiratory Therapist (RT), who has been nominated on the basis of professional excellence and compassion in the education and care of patients with pulmonary illness. RTs may be nominated for the award by patients, family members, visitors, and co-workers.
