Referral Policy

All new/returning patients, regardless of insurance, will require a referral from Primary Care, Urgent Care, or the Emergency Department. Have questions about becoming a new patient? Contact us by phone: 540-450-2339.

Please print, complete and bring these forms to your first appointment:

Please plan to arrive 15 minutes early to your first appointment with your completed new patient forms in hand. If you are unable to complete these forms ahead of your first appointment, please plan to arrive 30 minutes early.

Cancellation Policy

Our office prefers 24 hour notice of a cancellation. Please contact us as soon as you know you cannot make your scheduled appointment. Thank you!

Late Arrival Policy

Running late? Please let us know with a phone call. Arriving more than 15 minutes late may result in appointment rescheduling.
