Additional Support for Employees and Families When Facing Health Challenges

When dealing with healthcare challenges it can be difficult to get the care and support you need. This can be true whether it is a new injury or illness or a chronic condition. It is often true even for individuals who work in a healthcare setting.

As a benefit for our caregivers and their families, Valley Health has established a team of specialized nurses who can work closely with you to address your specific health needs. This free and confidential service is available for Valley Health employees and their family members who have medical insurance through Valley Health.

Nurse care coordinators can assist you in setting health goals and offer the support needed to help you reach those goals. This includes providing education and being a resource between doctor visits. The care coordinators will also help you access any needed health services including specialty care. This program does not take the place of programs like Healthy U, but is one more way we can be healthier together.
