This practice offers rapid, point-of-care testing for influenza, COVID-19 and strep, or a referral for such testing for patients with symptoms.

This practice is now offering telehealth virtual visits for patients. To make either an in-person or virtual appointment, please call our office.

Patient Information

Your first visit to Valley Health Family Medicine | Inwood establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.

We ask that you bring your driver's license or photo ID, insurance card(s) and any copay that may be due to your first appointment. If you are taking any medications, it is beneficial for you to bring your complete medication list or the bottles of your current prescription and over-the-counter medications. By providing us with this information, we are able to see you promptly and maintain an accurate record of information for you. If you would like to transfer your medical records to our office, please contact our office to request the transfer forms. Once the forms are completed and returned, we can move forward with obtaining your records and schedule an appointment for you.

We are pleased to offer walk-in care for your immediate healthcare needs via Valley Health Urgent Care Express, located within our practice. Additionally, for our patients, we offer onsite laboratory testing which is also available at our many Valley Health Urgent Care locations. We will not only take care of you for your immediate need but also if desired, we will schedule a follow-up visit. If you require a referral to a specialist, we will manage this process for you to ensure your path to wellness is successful!

Valley Health Urgent Care Express

Valley Health Urgent Care Express is also located within the practice and is open to anyone during our walk-in hours Monday-Friday from 8 am - 6 pm and Saturdays from 9 am - 5 pm. Valley Health Urgent Care Express can treat:

  • Flu-like symptoms
  • Allergy symptoms
  • Sinus congestion and infections
  • Sore and strep throat
  • Earaches
  • Upper respiratory infections
  • Pink eye and sties
  • Urinary tract and bladder infections
  • Minor cuts
  • Insect bites

  • Poison Ivy/Oak
  • Vaccinations
