This location is a COVID-19 test collection site by physician referral only. Your sample will be sent off for testing, and your results will be ready in about 48 hours.

15 Minute Rapid COVID-19/Flu/Strep Testing Available
This practice offers rapid, point-of-care testing for influenza, COVID-19 and strep, or a referral for such testing for patients with symptoms.
This location offers on-site COVID-19 testing by physician referral. Those who have been exposed to COVID-19 or have symptoms should call their primary care provider or the Valley Health designated COVID-19 hotline (540-536-0380) for referral.

Patient Information
Your first visit to Valley Health Berkeley Family Medicine establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.
We ask that you bring your driver's license or photo ID, insurance card(s) and any copay that may be due to your first appointment. If you are taking any medications, it is beneficial for you to bring your complete medication list or the bottles of your current prescription and over-the-counter medications. By providing us with this information, we are able to see you promptly and maintain an accurate record of information for you. If you would like to transfer your medical records to our office, please contact our office to request the transfer forms. Once the forms are completed and returned, we can move forward with obtaining your records and schedule an appointment for you.
