Valley Health’s Community Partnership Grant Program: Addressing Critical Health Needs

Valley Health is pleased to announce the findings of the 2025 Community Health Needs Assessment and to introduce the 2026 Community Partnership Grant program.

Valley Health completes a community health needs assessment (CHNA) every three years. The assessment is a systematic evaluation of our community’s overall health and is used to identify key health challenges and the resources available to address these challenges. The 2025 assessment was conducted in partnership with the Virginia Department of Health’s Lord Fairfax Health District and community stakeholders.

Based on the results of the CHNA, the following priority health needs have been identified across the region served by Valley Health:

• Access to Health & Wellness Services
• Nutrition & Physical Well-being
• Mental Health
• Housing

Working collaboratively with community partners, Valley Health and the health department will develop strategies to address these priorities for the current three-year CHNA period.

Community Partnership Grants

The Community Partnership Grant program is one strategy Valley Health uses to address the health needs identified through the CHNA process. Local organizations are invited to apply for one of 13 one-year grants by submitting an application detailing the ways they plan to partner on the identified health priorities. Valley Health proudly supports community impact partners that extend our mission of serving our community by improving health.

The 2026 application period for our Community Partnership Grant program is open now through February 27.

“We are proud to support and partner with community agencies to address the area’s critical needs,” said Jason Craig, Senior Director of Community Health for Valley Health. “Through this program, our hope is that our community partners can collaborate with the health system to alleviate the challenges many individuals face when it comes to accessing health care, housing, mental health care and more.”

The grant awards total $200,000 and are structured as follows:

• One grant award of $50,000
• Four awards of $25,000 each
• Four awards of $10,000 each
• Four mini-grants of $2,500 each

Grant recipients of the Community Partnership Grants will be announced in April 2026.
