Valley Health Virginia & West Virginia

Our team of clinicians includes nurses, physical therapists, occupational therapists, speech therapists and more—provide direct care inside of patients homes, allowing them to stay home without fear of missing out on vital care. We work with the patient and their physician to establish a Plan Of Care.

Our home health services include treatment and care for those who have experienced:

  • Alzheimer’s/Dementia
  • Cancer/Oncology
  • Multiple Chronic Conditions
  • Serious Illness (pneumonia, infection, flu, etc.)
  • Joint replacement or surgery
  • Falls
  • Osteoporosis/fracture
  • COPD or other respiratory conditions (asthma, bronchitis, pneumonia, and ventilator patients)

Care We Provide

Valley Health Home Health cares for patients with services including:

  • Direct care that includes teaching to patient and caregivers
  • Medication administration (including I.V. therapy and injections)
  • Wound care
  • Physical therapy
  • Occupational therapy
  • Speech pathology

Our Highly Trained & Qualified Staff

Valley Health Home Health is staffed by highly qualified, experienced healthcare professionals, including:

  • Nurses evaluate changes in physical condition, administer medications, change dressings and draw blood for testing, along with teaching patients and caregivers to effectively manage the patient’s health needs.
  • Physical Therapists evaluate rehabilitation needs, teach therapeutic exercises and identify and instruct on safety issues.
  • Occupational Therapists help maximize independence in activities such as bathing, grooming, toileting, dressing, etc.
  • Speech Pathologists assess and provide treatment for language and hearing disorders.
  • Social Workers evaluate social and emotional needs and refer patients to appropriate community resources.
  • Home Health Techs assist with activities of daily living.
