Valley Health Transition Clinic

The Valley Health Transition Clinic provides short term medical care. Our team of healthcare professionals will assist you with your healthcare needs and connect you to medical care in your community. 

What happens during the visit?

• Examination by a physician or nurse practitioner
• Discussion about your illness or injury and how to live a healthy lifestyle
• Assistance with your medication
• Assistance making appointments for tests you may need
• Assistance finding a doctor or healthcare professional in your community

Valley Health Patients are referred to the Transition Clinic.

• When discharged from a Valley Health facility without a primary care provider
• After an Emergency Department, ACCESS, or Observation visit without a primary care provider
• When unable to see their primary care provider within 7 days of discharge

Our goal is to provide treatment and education to keep you healthy. Patients are seen regardless of ability to pay or payment source.
