Your words matter. Gratitude encourages our caregivers, strengthens our community, and reminds our teams why they do what they do.

Ways to Give Thanks

Through one simple form, you can choose how you’d like to express your gratitude.

1. Share a note of thanks or a personal story

Write a personal note to an individual caregiver or an entire team who made a difference during your care. Your message will be shared with the caregiver, and with your permission, stories may be shared internally with staff or publicly to celebrate the compassionate care happening across Valley Health.

2. Nominate a caregiver for a recognition award

If someone on your care team went above and beyond, you may choose to recognize them through one of the awards below. Simply select “Nominate a caregiver for a recognition award” in the Give Thanks form and share your story. You do not need to complete a separate award form.

DAISY Award

The DAISY Award recognizes nurses who provide extraordinary, compassionate care. This international honor celebrates both clinical excellence and the meaningful connections nurses build with patients and families.

Carnation Care Award

The Carnation Care Award honors Certified Nursing Assistants (CNAs) and Technicians who consistently demonstrate exceptional care and compassion beyond expectations.

Sunflower Award

The Sunflower Award recognizes outstanding ancillary and support services employees whose work plays an essential role in patient care and the overall hospital experience.

PHIL Award

The PHIL Award honors a respiratory therapist who demonstrates professional excellence and compassionate care, particularly in supporting patients with pulmonary illness.

Not Sure Which Award Applies?

That’s okay. You don’t need to know which recognition program fits best. Simply share your message of thanks and indicate your interest in nominating a caregiver or team. Our teams will thoughtfully review and route submissions to the appropriate recognition program.

3. Make a gift in honor of a caregiver or department

You may also choose to make a donation in honor of a caregiver, team, or department. Gifts help support Valley Health programs, caregivers, and patient care initiatives.

Giving is always optional. 100% of every gift stays local.
