Valley Health Primary Care | Amherst, A Department of Winchester Medical Center, offers comprehensive primary care services to patients. This practice offers short-term, transitional primary care to patients who don't have immediate access to a primary care provider and are in need of one.

Our team of clinicians assist patients with their unique health needs and connects them to long-term medical care in the community.

What happens during the visit?

• Examination by a physician or advanced practice provider
• Discussion about your illness or injury and how to live a healthy lifestyle
• Assistance with your medication
• Assistance making appointments for tests you may need
• Assistance finding a doctor or healthcare professional in your community

Valley Health patients are referred for transitional care to this practice:

• When discharged from a Valley Health facility without a primary care provider
• After an Emergency Department, ACCESS, or Observation visit without a primary care provider
• When unable to see their primary care provider within 7 days of discharge
• Our goal is to provide treatment and education to keep patients healthy. Patients are seen regardless of ability to pay or payment source.

In addition to transitional care services, Valley Health Primary Care | Amherst is home to the Valley Health Internal Medicine Residency Program. Valley Health is proud to train the next generation of physicians. Through this program, residents receive expert-guided training while expanding access to care for the community.
