Diabetes Prevention Program: Info Session
Have you been diagnosed with prediabetes (higher-than-normal blood sugar)?
The Diabetes Prevention Program is a FREE program for adults at risk for
type 2 diabetes and/or heart disease. The program will help you take steps
to decrease your risk.

You may be eligible for this program if you do not have diabetes, are
overweight and have one or more conditions:
• Elevated blood sugar levels
• High blood pressure
• Large waist measurement
• Abnormal blood fat levels
• History of gestational diabetes
Recognized by the Centers for Disease Control and Prevention, this intensive
twelve-month lifestyle change program is designed for individuals ready
to make a lasting change.
