Long-term care is a combination of medical, nursing and many other services designed to help people who have disabilities and/or chronic care needs over a period of time, typically six months or longer. You may receive long-term care in a healthcare facility such as a nursing home or long-term care unit in a hospital. Some long-term care facilities may also offer short-term skilled care (nursing care and physical, occupational and/or speech therapy).
Three Valley Health facilities offer long-term care:
Hampshire Memorial Hospital
Lynn Care Center (Warren Memorial Hospital)
War Memorial Hospital
