Community Health Partnership Grants

Partnering for Healthier Tomorrows

Valley Health recognizes our community’s health concerns are complex and that we cannot solve them alone. We are proud to support and partner with area community agencies, allowing us to leverage our resources for more meaningful impact on persistent health trends and disparities affecting our region.

Valley Health’s Community Partnership Grant program provides one-year grants totaling $200,000 to area non-profit organizations that address critical health needs. For 2025, one organization will receive the top grant award of $50,000, four will receive $25,000 awards, four receive $10,000 awards and four receive $2,500 mini-grants.

Community Partnership Grant recipients are selected through a competitive application process, evaluating how proposed projects align with Valley Health strategies including education and navigation, mobile health, telehealth, transportation, the behavioral health continuum, and community collaborations.
