A Holistic Approach to Care
Lynn Care Center provides 24-hour nursing coverage under the supervision of the Director of Resident Services. The nursing staff includes registered nurses, licensed practical nurses and certified nursing assistants. The nursing staff focuses on overall medical and restorative care, taking a holistic approach to help residents understand their diagnosis, medication, diet and restorative plan.

Staffing
Lynn Care Center strives to staff at one nurse for every 32 residents and one certified nurse aide (CNA) for every eight residents. This staffing ratio meets or exceeds Virginia state regulations and is higher than staffing averages reported nationally by the Center for Medicare and Medicaid Services. We average 3.5 hours of nursing care per resident per day.

Specialized Nursing Programs

Lynn Care Center has specialized nursing programs and services to address resident's individual needs:

Restraint-Free Facility
The goal of Lynn Care Center is to be a restraint-free facility and to use diversional activities instead of physical or chemical restraints.

Fall Prevention
A comprehensive Falls Prevention Program includes and assessment of fall risk upon admission and then quarterly. Our Falls Task Force evaluates all residents who are at high risk for falls, plans interventions, and conducts an assessment should a resident experience a fall. The fall prevention plan is communicated to all staff caring for that resident. The Falls Prevention Program at Lynn Care Center has a proven track record in reduction of falls with higher than average results.

Restorative Nursing
We offer a variety of restorative nursing programs to maintain health of mind and body and to promote independence and quality of life.

Incontinence Management
One of the goals of our restorative nursing program is to help the resident maintain continence or manage incontinence. Our geriatric nurse practitioner works with individual residents, families and Lynn Care staff to promote continence management.

Pain Management
Our interdisciplinary team is committed to providing appropriate pain assessment, prevention, effective management, and quick responses to reports of pain to meet the resident's specific needs.

Therapy and other services offered right here when needed

We offer on-site therapy and pharmacy support, which includes licensed physical, occupational and speech therapists as well as a licensed pharmacist.

Specialty Units

Skilled Nursing Unit
Lynn Care Center offers short-term skilled stays with the goal of returning you to your previous living situation. We pride ourselves on having over 80 percent of our patients returning to the community. The average community return rate at most skilled nursing facilities is less than 50 percent.

Skilled nursing care includes: IV therapy, dialysis, physical therapy, occupational therapy, speech therapy, respiratory therapy, wound management, and TPN (total parenteral nutrition).

Memory Support Unit
At Lynn Care Center we offer memory support programs to focus on the resident's ability and strengths as he or she adjusts to living with dementia in order to be able to live as safely as possible.

The Memory Support Unit is a secure area to promote moving about in a safe, protected environment. We also provide a secure and interactive wander garden for residents living in the unit. Both recreational activities and activities of daily living focus on the resident's strength, preferences, and history.

We encourage family members to visit regularly and to participate in the daily schedule for their loved ones.

Warren Memorial Hospital sponsors a community support group for families dealing with Alzheimer's disease and related dementias. The group meets the second Monday of each month at Lynn Care Center. Call (540) 636-0138 for more information.
