At Lynn Care Center we are sensitive to a resident's need to feel a sense of control over his or her living environment. Therefore, we have designed one person to function as each resident's advocate and social worker. The social worker is the liaison between the resident, family and the interdisciplinary care team.

The Lynn Care Center Social Services Department attempts to identify and modify any social, psychological, and/or environmental factors which contribute to the health problems of our residents.

Shortly after admission, the resident and/or family will meet with social worker to obtain a social history. The history helps to build a relationship and evaluate interests, needs, concerns and, where appropriate, discharge plans. Social services are available for all residents/patients and their families to deal with any problems that occur and provide support when needed.
