Valley Health Palliative Care will provide a short presentation and then assist attendees in completing their Advanced Care Directive.

Advanced Care Directives are important for everyone 18 and older to complete. If you are incapacitated and unable to make health care decisions, an Advanced Care Directive tells your health care team what you want and don't want to be done for you, as well as determines who can make decisions on your behalf. This event and Advanced Care Directive services are free for all attendees. Providers will be in attendance to assist attendees in completing the Advanced Care Directive process.
