Payment Arrangements Payment arrangements can be made for larger bills that can not be paid off all at once.
The first step is to contact us to request a payment arrangement, including no-interest monthly payment plans. We will then review your account for the different options that we offer. In many cases, you will be asked to fill out a Financial Information Sheet (FIS). We can provide one, or you can print one out by clicking here. Please fill out the form and return it in a timely manner so we can review your circumstances. This will help us make a more informed determination/suggestion of your options for resolving your bill. Charity Assistance Charity Assistance is available for those patients who meet predefined criteria. The first step is to contact us to request a Financial Information Sheet (FIS) or go to our website to the link listed above. You must return your FIS timely so that we can review the individual or families’ information against the criteria. In most cases, additional documentation is required such as proof of income or a notarized letter of support. You must request this prior to your account being referred for collections. All of your information will be kept confidential and only used to determine possible assistance programs to resolve your healthcare bills.
