Your first visit to Valley Health Family Medicine | Inwood establishes
a vital foundation for our relationship with you. During the first visit,
we make sure to obtain important background information, like your medical
history, and give you time to get to know your doctor.
We ask that you bring your driver's license or photo ID, insurance card(s)
and any copay that may be due to your first appointment. If you are taking
any medications, it is beneficial for you to bring your complete medication
list or the bottles of your current prescription and over-the-counter
medications. By providing us with this information, we are able to see
you promptly and maintain an accurate record of information for you. If
you would like to transfer your medical records to our office, please
contact our office to request the transfer forms. Once the forms are completed
and returned, we can move forward with obtaining your records and schedule
an appointment for you.
We are pleased to offer walk-in care for your immediate healthcare needs
via Valley Health Urgent Care Express, located within our practice. Additionally,

We will not only take care of you for your immediate need but also, if
desired, we will schedule a follow-up visit. If you require a referral
to a specialist, we will manage this process for you ensure your path
to wellness is successful!
