Valley Health provides a wide range of products and services which enable patients to receive the care they need in the comfort of their own homes.
Valley Home Care provides home oxygen, medical equipment and supplies.
Valley Health Home Health Services and Community Nurse Case Management provide healthcare professionals including nurses, physical, occupational and speech therapists, home health aides and medical social workers.

Valley Health Home Health Services Home Health professionals work closely with the patient, his or her family and physician to provide a wide range of services.
These services include:
Nursing services Nurses evaluate changes in physical condition; administer medications; change dressings and draw blood for testing. High-tech treatments such as IV therapy and ventilator care are also provided. Registered nurses are available 24 hours a day, 7 days a week.
Therapy services
Physical therapists evaluate rehabilitation needs; teach therapeutic exercises, gait training, and prosthetic training; and identify and instruct on safety issues.
Occupational therapists help maximize independence in activities of daily living such as bathing, grooming, toileting, dressing, eating, and home management by assessing upper body functions, vision and cognitive skills.
Speech pathologists assess communication needs and provide treatment for language and hearing disorders.
Social workers evaluate social and emotional needs, and refer patients to appropriate community and financial resources.
Personal Care Services Home health aides offer personal care, assist with activities of daily living and perform household chores.
Valley Health Home Health Services care for patients with a variety of medical needs including:
- Cardiovascular disorders
- Diabetes
- Medication administration (I.V. therapy and injections)
- Oncology
- Respiratory disorders (asthma, bronchitis, pneumonia, and ventilator patients)
- Wound care
- Joint replacement
- Osteoporosis/fracture
- Infants & pediatric patients with ongoing medical needs
- Mother & baby post-delivery follow-up

Eligibility Home health services are available to anyone, provided that medical and social needs allow the patient to receive care in the home. The patient must also be under the care of an attending physician and their home should be located within the geographic service area. Service areas include the city of Winchester and the towns of Front Royal, Woodstock, and Luray, as well as Frederick, Clarke, Warren, Shenandoah and Page counties and surrounding portions of Loudoun, Page, and Rappahannock counties
Payment Medicare, Medicaid and most major independent insurance agencies cover services provided by Valley Health Home Health Services. The home health agencies provide direct insurance billing for patients. Patients will not be denied care based on their ability to pay.

Community Nurse Case Management The mission of the Community Nurse Case Management program is “to assist clients with special health needs maintain a high level of wellness and remain as independent as possible”.
Communities Nurse Case Management links the services of registered nurses with persons who have chronic illness and special needs. Through in-home visits, nurse case managers coordinate the care of these clients, promoting wellness and self-care, encouraging patients to function as independently as possible in the community.

Eligibility Clients with a range of illnesses or health conditions may be ideal candidates for Community Nurse Case Management.
Eligibility criteria include:
Referral Information Physicians, social workers, hospital or home health nurses, or even family members may refer candidates for Community Nurse Case Management, by calling (540) 536-5200. All referrals will be screened for the appropriate level of care.
Community Nurse Case Management is not intended to duplicate services provided by physicians, hospice, home health agencies or caregivers. Instead, services are designed to assist clients with chronic or special needs manage their health to avoid medical crises and frequent hospitalizations.
